Patients are usually sent by a dentist, a physician, or they come themselves after noticing a change that does not settle. The first visit is unhurried: history, examination of the mouth and neck, and a plain explanation of what the findings mean.

When the problem is cancer or a serious injury, planning includes reconstruction and recovery — not only the operation. When the problem is smaller, that is said clearly, so no one leaves carrying more worry than the situation requires.
